Steer #58 (live 2026-10-09 05:18): a parked queued-busy steer was leapfrogged by a newer one and never retried.

Root cause chain (live forensics, anchors 1791515932971/1791516042601/1791516155087): the fast lane's delivery spawn measured the LAUNCHER's exit — a fire-and-forget systemd handoff exits 0 at START — wrote a false delivered history record (found at ts 1791515970.96), and that record made the real dispatcher's exit-3 self-queue refuse to re-queue. The steer sat parked with no representation anywhere while the newer send drove the idle target directly (waiting=0 → no FIFO grace). The same false record fed the UI's delivered flag — the three contradictory states (disk queued-busy / UI FINISHED / transcript never received).

D1 orphan liveness: watcher re-queues parked queued-busy steers whose representation was lost (child + prompt verbatim), clears the provably-false bare-fastlane record (receipts arbitrate; dispatcher-shaped records respected), attempts sidecar (age + attempts honest). Dispatcher-side note: suppression is receipt-based (v1.0.32 ONCE) — a never-delivered retry is never swallowed, no cross-lane change needed.
D2 no leapfrog: waiting counts include parked orphans; boundary drains oldest-first (timestamp-proven in the three-anchor rig).
D3 honest UI (steers #59/#62): DELIVERED only from the send-ack receipt (or the dispatcher's completed result); queued-busy renders WAITING with age; local-first instant echo (zero roundtrip in the render path) surviving re-render churn; state-machine unit test covers every (disk, receipt) pair.
D4: S-40 one warning surface per view mode; S-41 loading-vs-loaded identical at 1177/900 (stamp slot reserved, chips compacted; one-row band + pre-regression header height restored).
XX extension: traceless dismissal dead-letters permanently.

Suite: 819 tests (only the pending-tag version guard until v1.60.0 tags). Rig: G4 CDP 13/13, S-40 5/5, S-41 2/2. Super-Linter slim v8.7.0 green.

churn; chips transition sending → queued/delivered per the receipt.

D4: S-40 one warning surface per view mode (collapsed header hides
the canary banner — the slim line carries it); S-41 loading-vs-
loaded geometry identical at 1177/900 (stamp slot reserved, chips
compacted — the loaded band keeps the loading skeleton's one-row,
beside-title geometry; no layout jump at the data swap).

Round XX extensions: dismissing a traceless parked orphan dead-
letters permanently (the liveness path never resurrects it); the
cleared-record exemption keeps repaired steers neither-delivered-
nor-dropped.
